Claiming Short Term Disability Benefits


Disability benefits are meant to help people who have been disabled and are unable to work and support themselves by offering them financial assistance. The benefits programs can be of several types to suit individual requirements such as the short term disability benefits.

Short term disability benefits are a type of insurance coverage which provides claimant with a part of your salary when you are unable to work for a temporary period of time. Short term disabilities that are included in the program are injuries, pregnancy or a serious medical affliction which prevents the individual from performing his/her work. Any of these situations can qualify them for short term disability benefits.

First thing one needs to do is obtain a short terms benefits claims form from an insurance provider. Once you receive it, fill it up with appropriate information that has been requested for in the form. Explain the exact specifications of the disability you are suffering from and also mention the first date that you were absent from work as a result. You are also required to reveal certain personal information such as your social security number, your complete contact information and details of any other claims that you have filed in the past for disability including worker’s compensation.

The short term disability benefits form also has a specific section to be filled up exclusively by your employer. Your employer is required to furnish information on how much you earn per week, your job title, the number of hours you spend at work, the kind of work that you do and the last date you showed up at the office.

You also need to get your form validated by a physician to be able to claim short term disability benefits. He or she is required to provide details regarding your diagnosis, any symptoms and past treatments. They are also required to fill in a short survey pertaining to your limitations as a result of the disability.

Once completed, you can send the form to your insurance provider, who may then request you for medical records and an authority to audit them. Once the claim has been successfully processed, you can look forward to receiving benefits in the form of part salary payments for a specific number of weeks that have been stipulated in the disability benefits claim.
